We do not sell your information to third parties. How We Use Your Information

Information We Collect from Consumers and Customers

Fidelity Life Association may collect nonpublic personal information about consumers and customers from the following sources:

  • Information we receive from affiliates of Fidelity Life Association.
  • Information we receive from an individual and his or her employer from reports summarizing his or her insurance applications, plan participation forms, and other related documents.
  • Information based on an individual’s history of transactions executed with the assistance of Fidelity Life Association, our affiliates, or others.

We may use the information you submit on this website to provide you with requested insurance information or to share additional products or services that could be of interest to you. If you do not wish to receive offers for additional products or services, you should not provide personal information or contact information through this website. If you have already provided profile data or contact information, you may opt-out from receiving special offers from Fidelity Life in the future.

We also require outside companies and independent contractors who provide services on our behalf or with whom we have entered a joint marketing agreement to enter into agreements that protect the confidentiality of any customer information we provide and restrict the use of such information to those purposes. Your decision to provide personal contact or medical information is strictly voluntary and will not prevent you from receiving a quote for life insurance. You may call us toll-free at 800.482.6616, Option 4. Agents are available Monday through Thursday from 8 a.m. – 9 p.m. CT and Friday from 8 a.m. to 7 p.m. CT. We will be happy to quote insurance rates and complete your application request by telephone.

If you elect to provide personal or contact information through this site, Fidelity Life may use or disclose the information you provide to trusted third parties. In addition, Fidelity Life, or the referenced third-party companies, may use the information to send relevant offers to you which we, or the third parties, believe would be of interest to you. If you wish to opt-out from receiving any offers from Fidelity Life, simply click the opt-out link here or call 800.369.3990 Monday through Friday from 8 a.m. to 5 p.m. CT.

Please note, that even if you select to opt-out after providing your information through our website, the third parties you’ve been in contact with may still contact you. If you wish to not receive marketing materials, emails or information from them in the future, you would have to opt-out directly from their websites. As a general recommendation, if you do not wish to be contacted at all, you should not provide any personal contact information through this website.

Unless you give us permission to do so, or unless the law requires us, we do not willfully use or disclose personally identifiable information to third parties for any purpose other than described above. We have security provisions in place to protect your information; however, no security system is impenetrable. Changes to FidelityLife.com’s Privacy Policy

We reserve the right to change our privacy policy, and if so, we will post those changes to our FidelityLife.com privacy statement that is linked throughout our website so that visitors are aware of the change. We encourage you to check this posted privacy statement for updates. When changes are made, we will post the last date on which the privacy policy was changed below. In addition, you may request the prior version of our privacy policy before the most recent change by emailing us at service@fidelitylife.com.

Usage of a cookie is in no way linked to any personally identifiable information while on our site. Fidelitylife.com may contract with certain partners that utilize ad-serving technology. To gauge the effectiveness of advertisements, a campaign number is assigned to the vendor for which you came to visit our site. This will remain on your URL when you return to our site.

In addition, we do use Google Analytics on our website as do many of our partners. Vulnerability Security Policy

Introduction

Vericity, the parent of Fidelity Life Association, is committed to ensuring the security of the American public by protecting their information. This policy is intended to give security researchers clear guidelines for conducting vulnerability discovery activities and conveying our preferences about how to submit discovered vulnerabilities to us.

This policy describes what systems and types of research are covered under this policy, how to send us vulnerability reports, and how long we ask security researchers to wait before publicly disclosing vulnerabilities.

We encourage you to contact us to report potential vulnerabilities in our systems.

Authorization

If you make a reasonable faith effort to comply with this policy during your security research, we will consider your research to be authorized. Should legal action be initiated by a third party against you for activities conducted according to this policy, we will make this authorization known. We will work with you to quickly understand and resolve the issue, and Vericity will not recommend or pursue legal action related to your research.

Guidelines

Under this policy, “research” means activities in which you:

  • Notify us as soon as possible after you discover an actual or potential security issue.
  • Make every effort to avoid privacy violations, degradation of user experience, disruption to production systems, and destruction or manipulation of data.
  • Only use exploits to the extent necessary to confirm a vulnerability’s presence. Do not use an exploit to compromise or exfiltrate data, establish persistent command-line access, or use the exploit to pivot to other systems.
  • Provide us with a reasonable amount of time to resolve the issue before you disclose it publicly.
  • Do not submit a high volume of low-quality reports.

Once you’ve established that a vulnerability exists or encounter any sensitive data (including personally identifiable information, financial information, proprietary information, or trade secrets of any party), you must stop your test, notify us immediately, and not disclose this data to anyone else.

Prohibited testing methods

The following test methods are not authorized:

  • Network denial of service (DoS or DDoS) tests or other tests that impair access to or damage a system or data
  • Physical testing (e.g., office access, open doors, tailgating), social engineering (e.g., phishing, vishing), or any other non-technical vulnerability testing
  • High intensity scanning that may affect the availability of Vericity’s systems and services.

Scope

This policy applies to Vericity Inc systems only. No third-party systems, service providers, or partners are in scope, e.g., Salesforce.

Any services not expressly listed above, such as any connected services, are excluded from the scope and are not authorized for testing. Additionally, vulnerabilities found in systems from our vendors fall outside of this policy’s scope and should be reported directly to the vendor according to their disclosure policy (if any). If you aren’t sure whether a system is in scope or not, contact us at security@vericity.com before starting your research.

Though we develop and maintain other internet-accessible systems or services, we ask that active research and testing only be conducted on the systems and services covered by the scope of this document. If there is a particular system not in scope that you think merits testing, please contact us to discuss it first.

Reporting a vulnerability

We accept vulnerability reports sent to security@vericity.com. Please encrypt the email using our PGP Public Key. Reports may be submitted anonymously. If you share contact information, we will acknowledge receipt of your report within three business days.

What we would like to see from you (Optional)

To help us triage and prioritize submissions, we recommend that your reports:

  • Describe the location the vulnerability was discovered and the potential impact of exploitation.
  • Offer a detailed description of the steps needed to reproduce the vulnerability (proof of concept scripts and screenshots are helpful).
  • Be written in English, if possible.
  • Your disclosure plans, if any.
  • Your desire for public recognition (include your name and LinkedIn profile, if desired)

What you can expect from us

When you choose to share your contact information with us, we commit to coordinating with you as openly and quickly as possible.

  • We will acknowledge that your report has been received within three business days.
  • To the best of our ability, we will confirm the existence of the vulnerability to you and be as transparent as possible about what steps we are taking during the remediation process, including on issues or challenges that may delay resolution.
  • We will maintain an open dialogue to discuss issues.
